COPAY & NO-SHOW POLICY
No-Shows take up appointment times that could be used by other patients.
If you need to cancel or reschedule your appointment, remember to please call us at least 24 hours in advance.
If you fail to call 24 hours in advance, we will charge your credit card a "No Show" fee of $100 for new patients and $50 for existing patients. This fee is not covered by insurance companies.
If you have a copay for office visits or allergy shots, your insurance requires that we collect it at each visit. While we can no longer bill for copays, there are still 3 ways to make your payment:
1. Pay at each shot or MD visit.
2. Prepay for several shots.
3. Use EZ-Pay: Just leave your charge card number & we will automatically collect your copays, saving you time & checks.

HIPAA regulations do not allow us to use standard email to discuss medical questions. Please respond to our invitation to our EMR portal, Elation Passport, and use that messaging system to send direct messages to Dr. Davidson. If you have not received an email invitation or need a new one please call our office.
PRESCRIPTION REQUESTS MUST BE FAXED VIA YOUR PHARMACY OR MAIL ORDER PHARMACY.
